A note about labelling: a reference such as 2.9.3 is to Theorem 2.9.3 (or Def-
inition 2.9.3, etc., as the case may be). The numbers for displayed equations and
statements, however, are put in parentheses, so that (4.2.12) refers to Equation
(4.2.12), for example.
